Life Care Planning is a holistic look at all aspects of an elderly person's life -- housing, medical care, transportation, social needs, daily life and activities, etc. -- and attempts to synthesize recommendations into a personalized elder care plan. Our office collaborates with Area Agencies on Aging, local senior centers and various other resources to help make sure each senior's needs are met. Our Life Care Planning Coordinator has more than 30 years' experience as both a caregiver and a registered nurse serving hospice, local hospitals, the homebound and community health services. She is available for consultations and to help coordinate the various aspects of a comprehensive life care plan.
